Print application and submit after completing to the Travel Services Office via campus mail or fax (210-562-6290).
Reportable Travel to Washington DC
For travel including the immediate metropolitan area of DC: Alexandria, VA, Arlington, VA, Bethesda, MD, Chevy Chase, MD, and Silver Springs, MD.
Submit the completed form to Travel Services via fax (210-562-6290).
