Disciplinary Guidelines for HIPAA Violations Attempting to obtain or use, actually obtaining or using, or assisting others to obtain or use protected health information, when unauthorized or improper will result in counseling and/or disciplinary action up to and including termination. The specific discipline administered will depend on the nature and severity of the violation. Action will be initiated by the appropriate School or operational area in accordance with the classification of an individual (i.e., classified, credentialed faculty, student, etc.) and if necessary, the requirements of the individual's licensing Boards. Below is a link to each School's disciplinary guidelines for faculty, residents and students. All classified and administration professional (A&P) employees are covered under the first link.
