OHR 2007-045 - Health and Wellness Survey

The Health Science Center is interested in the health and well-being of its faculty, staff, and students.  Last year the University sponsored a Wellness Fair through the excellent management of the School of Nursing.  The fair was well received.  As a result, a Health and Wellness Fair will be held on November 1, 2007, and managed by the School of Allied Health Sciences with active participation from the Graduate and the Dental Schools and the Schools of Medicine and Nursing.   

The Health and Wellness Task Force would like your input about your interest in health and wellness topics.  There are two sets of questions in the Survey.  The first set relates to what you would like to have offered during the Health and Wellness Fair on November 1.   The second set relates to educational sessions and workshops that will be provided to the campus monthly beginning September 2007 through August 2008.   

The Survey should take no more than 10 minutes to complete.  This Survey is anonymous and data will only be used in aggregate.
