OHR 2008-010 - Current Home Address

It is critically important that the Health Science Center has a way to mail information to you through the U.S. Postal Service.  As we near the end of the 2007 tax year, the Offices of Human Resources and Payroll Services must have your current home address in order to send your W-2 Form needed for filing a 2007 income tax return. 

Please take a moment to check your information on the Employee Self Service on inside.uthscsa.edu to make sure that your home contact information is current and correct. 

If it is not correct, please update it. Please note: if you live in an apartment, the apartment number should be entered by itself on address line 1 and your street address on line 2.  If you do not have an apartment, enter your address on line 1.
