First Day Forms
Please print and fill out the following documents and have them with you when you report to Human Resources on your first day. Please date and sign the form when prompted. If you have any questions, please feel free to call our offices at (210) 567-2600. (Return to New Employee Guide)
- Employment Eligibility Verification (I-9)
- Review the lists of acceptable identification and work eligibility documents. Complete Section 1 only. Bring the form along with the required identification and work eligibility documents.
- Direct Deposit Authorization Form
- When completing the form you must attach a voided Check or Deposit slip that contains both the routing number and checking account at the bottom of the check or deposit slip. If you select direct deposit for Debit only, no voided check or deposit slip is required. You must have a badge ID already established before the form is turned in.
- Employee Witholding Allowance Form (W-4)
- You must complete line Items 1, 2,3 and 5. Item 6 is optional. See exceptions below:
- if there are no allowances for # 5
- if #5 is completed, then #7 must be blank
- if #7 is exempt, then #5 must be blank
- You must complete line Items 1, 2,3 and 5. Item 6 is optional. See exceptions below:
- Prior State Service Verification
- If you are not sure if you have ever worked for a state agency before, complete the form and HR will validate. If you have several state agencies, you will need to complete a form for each state agency you worked under.
