Mandatory Requirements for Faculty and Staff

When separating from the UT Health Science Center, the following steps should begin upon notification of your separation and must be completed no later than your last day of employment.

 

Research and Lab Faculty and Staff - Additional steps may be required for individuals involved in Research. Refer to the Research section below for detailed information.

  1. Inform your supervisor or department administrator of your separation.
  2. Update your personal information.
    • Verify that your home address, e-mail address and phone numbers are up to date by logging into Employee Self-Service or by sending an email to hr-admin@uthscsa.edu. It is important that we have your correct address on file to ensure you receive any post- employment documents.
  3. If you are a TRS participant and wish to cash out or rollover your funds, complete the Notice of Final Deposit and Request for Refund Form (TRS6).
  4. Return Physical Resources/Assets – On the last day of employment, return the following items to your department.  Note, all items included in the list below may not apply to each separating employee, and this list is not all inclusive.
    • Access Cards
    • Uniforms
    • Tools & Equipment, including radiation dosimeters
    • Mobile Devices
    • Laptop/Computer/iPad
    • Pro Card/Fuel Card
    • University Records
  5. Return Identification Badge, Keys and Parking Permits.  Keys include desk, file cabinets, office, building, vehicle and any other institution keys. Work with your department to determine the keys that should be collected by your last day of employment.

Employees who owe fines or have missing keys must report to UT Police to clear on their last day of employment. Otherwise, all items can be turned in to the deparment.

Note: If the employee returned their parking permit, they may request a temporary permit at any of the security gates on their last day of employment.

Research and Lab Requirements:

This section pertains to faculty and staff members involved in research which may include human subjects, laboratory animals, FDA regulated test articles, internal or external funding, radioactive, chemical or biohazardous materials.

 

When separating from the UT Health Science Center, contact the following offices, as applicable. Do this as early as possible to ensure adequate time to complete any necessary actions. You should communicate with your department once the applicable offices have been notified and the respective requirements are completed.

Environmental Health & Safety

Have you ever served as a principal investigator of a lab?

Are there any radioactive, chemical or biohazardous materials for which you are responsible?

For principal investigators, contact EHS for proper decommissioning of the laboratory. Please refer to the Decommissioning Policy. In addition, ensure all equipment, such as radiation dosimeters, is returned to your department.

 

To request safety records (training, dosimetry, exposure monitoring, etc.), complete the Request for Safety Records Form and fax it to 210-567-2965.

Institutional Animal Care Program

Have you ever served as a principal investigator for an animal research study?

Institutional Review Board

Have you ever served as a principal investigator for a human research study?

If so, contact the Institutional Review Board office by e-mail: irbmail@uthscsa.edu or by phone: 210-567-8250.

Lab Animal Resources

Do you have any research animals being housed?

If you have any research animals being housed, please contact Lab Animal Resources at 210-567-6166.

Office of Clinical Research

Have you ever served as a Principal Investigator on a project involving human subjects?

For those who have served as a Principal Investigator or staff for a study involving payments to research participants, investigational test articles or www.clinical trials.gov, or other study related responsibilities, contact the Office of Clinical Research or call 210-567-8555.

Office of Sponsored Programs

Are you a faculty member that participates in an active sponsored award?

If you are a principal investigator and/or faculty member that has committed effort on an active sponsored award, please have your department administrator contact the Office of Sponsored Programs about your separation. Separation of non-faculty members does not require notification regardless of their participation on sponsored awards.
