The free flu shots schedule is listed below.
• Wednesday, Nov. 2 from 12:30 to 5:30 p.m. in the Academic & Administration Building, Room 116
• Friday, Nov. 4 from 7:30 a.m. to noon in the Academic & Administration Building, Room 104
• Tuesday, Nov. 8 from 7:30 a.m. to noon in the Academic & Administration Building, Room 104
• Thursday, Nov. 10 from 1:30 to 5:30 p.m. in the Academic & Administration Building, Room 104
• Tuesday, Nov. 15 from 8 a.m. to noon at North Campus, CCRI Building 2nd Floor Lobby
• Wednesday, Nov. 16 from 8:30 a.m. to noon at the Texas Research Park Campus, Hayden Head Room 3.046
What to do to get an HSC flu shot:1. Evaluate the health risks and benefits of the influenza immunization. Click on
www.uthscsa.edu/HSCNews/archive/flushot.pdf for the flu vaccine information sheet.
2. Print out and complete the University Physicians Group (UPG) Influenza Vaccine Consent Form:
www.uthscsa.edu/HSCNews/archive/Flu_Shots.pdf 3. Attend one of the flu shot clinics provided by UPG.
4. Since flu shots are free to HSC faculty, staff and students, you must have your HSC identification.
5. Bring your completed Influenza Consent Form to one of the scheduled flu shot clinics.
