Forms for Reporting Student Progress

 

To monitor and record progress made in the M&I graduate program, students are required to submit appropriate forms. As shown in the following table, these forms are submitted to COGS and/or the GSBS at years during the pursuit of their degree. Students are encouraged to type in requested information into the active fields of the forms before submitting them.
