Evidence of Insurability applications available June 1-29
With Open Enrollment just around the corner, the office of human resources reminds employees that the time to file a new Evidence of Insurability (EOI) form is June 1-29. Employees who want to change their benefit plan selections for fiscal year 2001-2002 may need to complete an EOI application during this time. Insurance companies use the EOI process to document an employee's medical history and to determine if the employee is "in good health." Employees are required to provide Evidence of Insurability if they intend to increase their life insurance coverage, add long-term disability coverage, and/or add dependents to life insurance coverage or to any U. T. self-insured medical coverage. This applies if the dependents were eligible but not enrolled during the employee's eligibility period.
Insurance plans that do not require EOI are Delta Dental, United Dental and Superior Vision.
