Collegiate Volunteer Program
We now have a collegiate volunteer /shadowing program which has become more formalized at the UTHSCA.
This program is designed to give college students interested in pursuing a medical/health related career an opportunity to gain clinical, research, and professional experience, ultimately giving them insight into the health careers and possibly making them more competitive applicants into professional programs.
All students coming on campus:
- must be at least 18 years of age,
- must pass a background check,
- have a badge,
- and parking permit
Below are forms that students interested in volunteering/becoming a Mentee can print/copy. Please complete the forms and then fax or mail the forms to Dr. Chapa, Ph.D. for proccessing.
The first is the 'Person of Interest' form (pdf) allowing the UTHSCSA to conduct a background check.
The second form is an interest form (Word doc) so that we can "match them" to a mentor that suits your interest.
* Please note that it typically takes a month to process an application and match students with mentors, so APPLY EARLY!
UTHSCSA Faculty and Staff interested in becoming Mentors can please fill out the form below. Dr. Chapa will be in contact as soon as a student mentee has been assigned.
