Medical Records Request
The Health Information Management department is responsible for medical records, coding, tumor registry, dictation, transcription, and release of medical information. There are 2 HIM locations, one at the Medical Arts & Research Center building and the other at the UT Health San Antoni o Cancer Center building.
The medical record primarily serves the patient; however, it has many additional values to the medical care team and the UT Health Cancer Center administration. The most obvious use of the medical record is to document and review patient care encounters. Patient care providers rely on the medical record as the principal means of communication and information exchange regarding patients under their care.
Each patient has one legal medical record of which there may be multiple volumes. The record is maintained and retained by Health Information Management.
If you require copies of your medical records, the Request Authorization for Release of Personal Health Information must be completed and faxed to 210-450-6058.
Email is not considered a secure way to distribute the private information that may be found in a medical record. If you would like us to release your medical records to you through email communication, you also must complete this Request Authorization for Release of Personal Health Information by Email document.
