Medical Records Request
The Health Information Services department is responsible for medical records, coding, tumor registry, dictation, transcription and release of medical information.
The medical record primarily serves the patient; however, it has many additional values to the medical care team and the CTRC administration. The most obvious use of the medical record is to document and review patient care encounters. Patient care providers rely on the medical record as the principal means of communication and information exchange regarding patients under their care.
Each patient has one legal medical record of which there may be multiple volumes. The record is maintained and retained by Health Information Services.
If you require copies of your CTRC medical records, the Request Authorization for Release of Personal Health Information must be completed and faxed to 210-450-5537.
Email is not considered a secure way to distribute the private information that may be found in a medical record. If you would like us to release your medical records to you through email communication, you also must complete this Request Authorization for Release of Personal Health Information by Email document.
