UTHSCSA departments may submit their requests for telephone services using this online Service Request Form (SRF). No other paperwork will be required. There are some unique features departments should be aware of before using this form. Please read them below before proceeding to the form.

Unique Features:

• "EMAIL" Block -
  Insert your email address. When you submit the form an email message is generated to you confirming your request.
• "AUTHORIZED BY" Block -
  This is for the name of the person authorizing the work. It MUST be an Authorized Signature for the Account. T&N WILL VERIFY THIS AUTHORIZATION BEFORE WORK WILL BEGIN.
