If there are any issues, complaints or feedback regarding the department, please contact the Associate Dean for Academic and Student Affairs, Dr. David L. Henzi at email@example.com or 210-567-4818. In addition, complaints may be addressed directly to the Institutional Compliance Office as described at ComplianceLine - Hotline and Institutional Compliance Program.
Issue, Complaint and Feedback Policy
Any issues or concerns which fall outside of UT Health San Antonio’s program or institutional due process policies should be addressed to the Associate Dean for Academic and Student Affairs. The Associate Dean for Academic and Student Affairs will investigate any allegations and provide a report to the individual addressing the issue or concern. Concerns or complaints may also be directed to the Institutional Office of Regulatory Affairs and Compliance at http://uthscsa.edu/compliance/.
UT Health San Antonio expressly forbids anyone to take any form of retaliatory action against any member of the UT Health San Antonio community who in good faith voices concerns, seeks advice, files a complaint or grievance, testifies or participates in investigations, compliance reviews, proceedings or hearings, or opposes actual or perceived violations of UT Health San Antonio's policy or unlawful acts.
