Long-term and Rehabilitative Care

Specialists at UT Health San Antonio provide care at three long-term care or rehabilitative care facilities in San Antonio. Rehabilitation care or skilled nursing care includes rehabilitation and/or nursing care needed by the older person for a limited period of time following an injury or illness. Nursing home or custodial care is needed for people who are losing their ability to function independently due to dementia, chronic disease and/or increasing frailty. Our geriatrician and nurse practitioner teams are in these facilities every week.

Assisted living facilities

Assisted living facilities offer help with activities of daily living (personal or custodial care), but no, or very limited, medical care. They provide only personal assistance that can be performed by someone with little or no medical training. Our teams are in these facilities periodically depending on the medical needs.

Rehabilitation care or skilled nursing facility

Rehabilitation care or skilled nursing care includes rehabilitation and/or nursing care needed by the older person for a limited period of time following an injury or illness. Our nurse practitioner/geriatrician team typically sees patients in the facility every two weeks or so unless there is a specific medical need to see them sooner.

Nursing home

Nursing home or custodial care is needed for people who are losing their ability to function independently due to dementia, chronic disease and/or increasing frailty. Our geriatrician and nurse practitioner teams are in these facilities weekly.  After admission, our nurse practitioner and geriatrician team typically sees patients in the nursing hone every eight weeks or so unless there is a specific medical need to see them sooner.
