Request Work Online

Minimum system requirements required to submit requests for work:

• UTHSCSA domain login
• Microsoft Office Infopath 2003 0r 2007
  This is part of the Microsoft Office suite (Word, Excel, etc..) of programs. Consult your IT department if you do not have this program installed.

• Internet Explorer browser version 7 or later
  In order to launch the forms properly, certain settings changes need to be made in your Internet Explorer browser. Click here to launch the UPK tutorial.

Learn how to fill out the forms by using User Productivity Kits (UPK)

• A UPK allows you to view the steps involved in filling out the forms based on different learning modes of 'See It , Try It , Know It and Do It' using actual screenshots of the forms. The kits can be accessed using a web browser.
• 3 UPKs have been written to guide you in:
  • Making settings changes to your Internet Explorer browser
  • Submitting an SRF
  • Submitting a RAP

Report A Problem	Use the RAP to report incidents related to the maintenance, operation, and upkeep of HSC facilities. These incidents may be related to: Temperature or Ventilation (office too cold/hot) Electrical (burnt light bulb, tripped plugs, etc.) Plumbing (water leak) Structural (rain leak, stalled elevator, etc.) Custodial (general cleaning) Click here to launch the UPK for the RAP
